TOP 10: Latest Collaborative Care Agreements in Healthcare
Several health systems have formed new relationships to share the best practices and drastically cut healthcare costs. Here are the top 10 most recent collaborative care agreements in the United States.
10. Humana and Health Choice Utah Preferred Network
Based in four counties in Utah, coordinated care services will be available to Humana Medicare Advantage members in Salt Lake, Davis, Weber, and Utah County.
9. Aetna Whole Health Product
Aetna and Baylor Scott & White Quality Alliance have formed a collection of benefit plans designed for a coordinated care model in Texas.
8. Anthem Blue Cross and Blue Shield and Mount Carmel Health Partners
A commercial accountable care organization in central Ohio has been formed by Mount Carmel Health Partners and Anthem Blue Cross and Blue Shield. Both organizations will analyze data on quality, costs and utilization to help identify opportunities to improve care.
7. Florida Blue and Baptist Health
Health insurer Florida and Baptist Health Care are forming an accountable care organization for patients in northwest Florida. Launching on Oct. 1, physicians from the Baptist Medical Group will be rewarded for improving their patients’ health while reducing costs.
6. Vidant Health, Wake Forest Baptist & WakeMed
The yet-to-be-named company is a joint venture between the three partners, which will allow each to benefit from cost savings as well as share the best practices. The systems will continue to remain independent.
5. Aetna and Weill Cornell Physicians
Aimed at improving health outcomes for roughly 9,000 of Aetna’s commercial and Medicare members in New York, 1,237 physicians in the Weill Cornell group will help coordinate care for Aetna members.
4. Humana and Iora Health
The partnership between these two organizations will provide coordinated care for Humana’s Medicare Advantage members at four new Iora Primary Care practices. Two will be based in Phoenix, Arizona with two more in Seattle, Washington.
3. Puget Sound High-Value Network
CHI Franciscan Health, Virginia Mason Medical Center and other medical centers have formed a health network that will be managed by First Choice Health Care. The network will function as an accountable care organization for employers and will include eight hospitals, 163 clinics, 24 ancillary provider locations and 2,875 physicians. The network is available to self-insured employers with 50 or more employees.
2. Advocate NorthShore Health Partners
Two of Chicago’s largest hospital systems – Advocate Health Care and NorthShore University HealthSystem – approved plans last week to combine and create a 16-hospital, $16.8 billion system. The health system will be the largest in Illinois and 11th largest nonprofit in the country.

Anthem Blue Cross recently partnered with seven other major California health care systems to form a new provider network and health plan. The plan will resemble an HMO and will become available to large employers in 2015.
